The Community Group Family Medicine Center is proud to be in the process of becoming a Patient Centered Medical Home (see below). We are striving to put our patients’ needs first! We provide full spectrum family medicine care to patients of all ages, from newborns to the elderly.
Our Family Medicine Center is a modern, state-of-the-art facility located just five miles from Community Hospital East. The center spans 16,000 square feet and includes 30 exam rooms and two procedure rooms.
Our team of providers includes faculty physicians, resident physicians, registered nurses, medical assistants, clinical techs, billers, front office staff, referral specialists, as well as pharmacists and social workers. Faculty physicians serve as excellent role models to our resident physicians, seeing patients side-by-side with the residents and are always readily available in the office for questions.

Welcome to your patient-centered medical home!
What is a patient-centered medical home? It’s a model of care where you will have greater access to your personal doctor who coordinates your care through all stages of life, no matter your age or specific needs. In a patient-centered medical home, YOU are the center of the healthcare team. Your team is headed by your primary care doctor and includes all the healthcare specialists who help coordinate your care. All team members communicate through a shared electronic medical record system called MyChart, which you can also access to review your health history, check test results, and more. Insurance
We accept most insurance plans, including Medicaid. We offer sliding fee scales and some discounted services. Please call us to speak to an onsite billing representative for more information.
After hours care
When you need care after regular business hours, please call 317-355-5717 and follow the prompts to be transferred to our after-hours operator. The provider will be contacted and call you back as soon as possible to provide you with the next steps in your care.
